1. ASSUMPTION OF RISK
Parent/Guardian acknowledges that participation in soccer and related activities involves inherent risks of injury, including but not limited to sprains, fractures, concussions, and other physical injuries. Parent/Guardian voluntarily assumes all risks associated with the Player's participation in Apex Elite United FC activities.
2. RELEASE OF LIABILITY
Parent/Guardian hereby releases, waives, and forever discharges Apex Elite United FC, its coaches, staff, officers, volunteers, and agents from any and all claims arising out of or related to any injury, loss, or damage sustained by the Player during participation in Club activities.
3. MEDICAL AUTHORIZATION
In the event of a medical emergency, Parent/Guardian authorizes Club staff to seek immediate medical attention for the Player. Parent/Guardian accepts financial responsibility for any medical expenses incurred.
4. CODE OF CONDUCT
Player and Parent/Guardian agree to support the Club's standards of sportsmanship, respect, and fair play. Disruptive or disrespectful behavior may result in suspension or removal from the Club.
5. PHOTO & MEDIA RELEASE
Parent/Guardian grants the Club permission to use photographs, videos, and other media featuring the Player for promotional and organizational purposes, without compensation.
